High riding implants, what is wrong? (Photo)

I had silicon implants under muscle 8 months ago, Mentor 325cc. I was unhappy with the look (high riding from day one). My breasts were only firm on the top (where the muscle lays) but not on the sides where I could feel the implant. 3 weeks ago I had a revision and my surgeon removed the capsule but my breast look EXACTLY the same :( Do I need another revision and what could possible be wrong? Why is my muscle so hard but the implants are soft on the sides where I can feel them.

There is an inverse relationship between the nipples and the top of the breasts.  Your nipples are low on the breast mound and almost have a downward tilt.  At a minimum, you need to have your implants lowered.  This may or may not be possible because you may, in fact, have a tight inframammary fold.  If so, either you need to put in smaller implants or move the nipple areolar complex up, or both.  With reference to the capsular contracture that was operated on by your surgeon, some remnant of the capsule may still be present or, if non-pathogenic bacteria is the problem, you may have re-encapsulated once again.  At any rate, it's time to represent your problems to your Board Certified plastic surgeon.  He or she will have the best answers as to what you should do going forward.

Thank you for your question and pictures.At 3 weeks, it is too soon to tell. It usually takes time for implants to drop, at least 3 months.

From the photo, it looks as though the implants are fine.  The areolae are below the imframmary fold and will require repositioning for a better result.

a revision usually resolves it quite easily since you are seeing what the problem is and know what needs to be released.  You need to decide if your upper poles are good where they are or if you really want less fullness on top.  If good, you have to consider a lift.  If not good and you were okay with a more natural look, your fold must be lowered and you must accept a lower breast on your chest.  It would be nice knowing what kind of capsule your surgeon found at your revision.
