I am returning to my original surgeon for a revision and to switch from 715 cc moderate profile silicon to 800 cc high profile silicone. I asked if her would use an internal bra on me to give additional support. He said he is going to cauterize the pocket. This sounds 'old school' to me. Can anyone explain cauterization to me and why he wouldn't do an internal bra techniqe instead?

I think that the best person to ask about your implants would be your surgeon. What was removed and what was replaced will assist in answering your question.
I think that you will find that your areola will remain the size that they currently are ( or become slightly larger as the healing process continues); unfortunately, they will not go back to their previous size. Best wishes for an outcome that you will be pleased with long-term.
