Would I benefit from a BBL revision to treat this fat pouch and unevenness?

There are several facts that are unpredictable when it comes to liposuction and in particular, fat transfer. It takes good year, possibly longer for result to settle and mature. Only at that time consideration should be given to second stage which will in your case include combination of liposuction and fat grafting. In person consultation with your surgeon is recommended. Before and after photos would be very helpful. However you do note that your result was just “okay”. If so, you may in good company with many others who had high hopes for BBL but instead received disappointing results (lumpiness, unevenness, lack of projection in the central and/or lower buttock, post-deflation sagging, etc.) as most of the fat melts away. It is not quite as much the quality of the doctor nor liposuction technique but rather the quality of the procedure that is a large part of the problem. Thus, going back for round 2 BBL most often produces the same disappointing results but at another financial loss. Buttock implants indeed provide a more reliable, predictable, relatively safe, and permanent result, especially for adding projection to the buttock and often smoothing out lumpiness. Occasionally a lower buttock tuck may be advantageous for lower buttock fold asymmetries and/or sagging. Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in buttock implants so be sure to consult with one that is reputable and experienced. Unfortunately you may have to travel long and far away from home to find this particular surgeon. Glad to help.

I typically view the torso as a single anatomic unit and treat the entire torso when doing Liposuction. Not doing so tends to leave people with it unnatural fat distribution that typically doesn’t look normal. my analogy is painting three sides of a house. if you’re gonna paint the house you should paint all the sides. Likewise, if you’re going to reduce the fat on the torso you should do it on all the surfaces so that the patient has a normal fat distribution. Simply treating the problem areas makes people look partially treated.

Some peoples buttocks have connective tissue that limits projection and shape and this can create a flat spot on the lower inner buttocks which is worsened with doing a fat transfer. patients will often blame the surgeon for not having put the fat in the right place when they see the results later.
