I had the procedure done a year ago. My history is that I had a chin implant removed with disastrous consequences. My mentalis muscle is "severely damaged " according a new doctor who did his best to reattach the muscle superiorly. So, would another deeper V-Y lip procedure or even a V incision closed zipper-like help even a little bit?? When I pinch my lower lip together slightly I get a lift in height. Maybe it doesn't work like this? Ugh this is beyond frustrating!
Answer: Can my lower lip be lifted? The upper lip comes down as well as the lower lip with age. The consequences are the disappearance of the upper teeth and more visible lower teeth. This is one of the signes of aging. You could benefit from an upper lip lift. Few things can raise the lower lip: A facelift concentrated on the mid face can help raise the lower lip. Lower lip augmentation can help. A full thickness V excision is probably the most effective but it will leave a potential visible scar in the midline. A V-Y advancement from inside the mouth would mostly make the vermilion fuller and could help, partially acting like a lip filler. The other injectable option to consider carefully is Botox or Dysport to the DAO and even possibly to the DLI muscles that pull the lower lip down. Paralyzing these muscle will raise the lower lip but may have other consequences that would have to be carefully considered. I don’t think the mentalis muscle has much to do with your lower lip coming down. The reason your lower lip came down after the chin implant removal is the shrinkage of the implant capsule and loss of support.

Answer: V-Y plasty With close to 30 years of experience with doing V-Y plasties of the lip and popularizing this procedure combined with fat transfer (FATMA) I can tell you that I’ve realized that V-Y plasties without proper chin support are limited in their scope to elevate the lower lip and hide the lower teeth. I’ve come to realize the vital link between the chin and the lip almost to the point that I consider them as one unit. A strong chin is important to support the lower lip and those with weak recessive chins tend to age with their lower lips sagging and rolling forward. Therefore, if you have had a chin implant and subsequently had it removed and especially though the oral route you may need more than just a V-Y to elevate and firm up your sagging flaccid lower lip. Trying to stabilize the mentalis muscle onto the bony chin is not always a reliable procedure especially without an adequately projected chin. Therefore, these factors have to be considered before proceeding with any further surgery whether it includes a new V-Y plasty or not. There is also ways to support the lower lip with filler with Caisson Contouring, a specialized method of placing filler utilizing engineering principles. I hope this helps, best of luck !
