Answer: Alarplasty: things you should know Thank you for your question and photos. You appear to be a candidate for alarplasty, a procedure that focuses on altering the base of the nose. Excess tissue can be removed to reduce the appearance of your nostrils and create a more symmetrical and natural-looking result. Correcting nasal asymmetry can often improve breathing issues as well. Keep in mind that nose surgeries, such as alarplasty, are the most complex of all plastic surgery procedures, requiring experience, skill, and even artistry to achieve an optimal outcome. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon, with expertise in alarplasty techniques. They will be able to assess your concerns and determine the best course of action.

Answer: I would advise alar rim retraction. Your nose features a severely retracted alar rim and hanging columella. Visible nostrils can be improved by correcting alar rim retraction. Surgery is not an easy operation, but the surgical method is alar cartilage realignment and alar rim graft using ear or septum cartilage +/- composite graft, and a lot of improvement can be expected. Correction of hanging columella is also required. Hanging columella correction consists of caudal septum trimming and repositioning of medial crura of alar cartilages.

Answer: Rhinoplasty surgery may be considered for the repair of a hanging columella, and improved breathing. Based on your photos, you appear to have a wide and flaring nasal tip appearance. This also appears to be associated with a long nasal septum, a hanging columella, and excessive nostril show. Well performed rhinoplasty surgery may be considered to both improve your nasal breathing and to Narrow the base of your nose and achieve less nostril show. Please consult with several reputable rhinoplasty specialists who offer computer Imaging so you can see what might be best for you moving forward. I hope this helps! Sincerely, Dr Joseph

Answer: Revision Rhinoplasty Thank you for your question. A large portion of my practice is dedicated to rhinoplasty and revision rhinoplasty surgery. Biggest issue you have is the arched alar rims and the prominence of your lower septum otherwise known as caudal septum. In your case perform a caudal septal excision, alar rim grafting, and alar base (Weir) excisions to improve the disparity and make your nostrils significantly smaller. You should be able to have significant improvement. Best wishes to you.
