Thank you for your question. A large portion of my practice is dedicated to rhinoplasty and revision rhinoplasty surgery. Biggest issue you have is the arched alar rims and the prominence of your lower septum otherwise known as caudal septum. In your case perform a caudal septal excision, alar rim grafting, and alar base (Weir) excisions to improve the disparity and make your nostrils significantly smaller. Alarplasty is a very fine and quick procedure that can narrow the nostrils and reduce the flare to soften the appearance. It has other names as well like ‘Weir Excisions’ or ‘Nostril Reduction’.
