Tip Revision - can my tip be derotated and look like it did before? (Photo)

Question

I have yet to find a doctor who can bring the tip infratip lobule back and round out the columnella. Most pics I see patients with flat columnellas. This may be the aesthetic but it doesn’t look right on some. Can my tip be derotated and look like my before again?

Hello! Thank you for your question. As far as I can see from your photos you have still a dorsal hump and deprojected nose tip (if the first photo is your current nose.). For gain more projected tip and curved dorsum I recommend you revision rhinoplasty surgery. If you can provide photos with better lighting, from the side of your face and 1 meter away I would be able to make a proper evaluation. It would be more convenient to decide with face-to-face consultation. Best Regards!

Generally, tip derotation requires septal extension grafting. Based on your before and after photos, you do not appear to have an over rotated tip appearance, and you have pleasing repair of your hanging columella. I hope this helps! Sincerely, Dr Joseph
