A low carb diet and exercise will help break down any intra abdominal fat, which is the fat under the abdominal wall muscles. Each one needs to be evaluated by a plastic surgeon to determine the procedure that will result in the best outcome. In some cases liposuction alone will be indicated especially if the skin is not lax or redundant. However, should convexity of the rectus abdominis muscles be present, this will need to be addressed as fat removal alone will not result in a flat abdomen.
