I currently have saline mentor implants filled to 750. I am 5'4 117 lbs. How can I get cute medium sized Barbie doll boobs?

If you want to have smaller breasts, you will need to remove your existing implants and replace them with smaller ones. You will almost certainly require a mastopexy as well. You will be a difficult case to be certain. 750 cc implants are quite oversized for your height and weight. Your breast skin envelope is likely stretched and quite thin. This will make a mastopexy and implant exchange difficult. I would suggest removal of the implants first. Allow the skin envelope to recover for about 1 month or so. And then proceed with a breast lift. After the lift has healed, you could replace the implants with smaller ones. There are other ways to approach your situation, but that would be my suggestion. 

It's hard to say without seeing you, but I commonly have women go down in size.  A higher profile implant and sometimes a lift is needed to keep the breast aesthetics nice.

You will need to ask your surgeon these specific questions.Generally, patients can be down-sized, but the surgery is more involved that a routine breast augmentation. If a mastopexy is planned you must be sure you can live with the breast scars created from a mastopexy.  Pre-operative percutaneous deflation of saline implants in this setting can be useful for planning and possible skin contraction.Best wishes.

It is difficult to give you good advice without viewing pictures or without examination. Based on your description however my initial thoughts would be that you will benefit from downsizing of breast implants, careful selection of the breast implant size/volume that will help achieve the rounder look you are looking for, breast lifting surgery, and possibly the use of acellular dermal matrix for additional support. I would suggest that you select your plastic surgeon carefully. Ask to see lots of examples of his/her work helping patients achieve the outcomes you will be pleased with.  Then, it will be important for you to communicate your goals carefully. For a woman your size, 750cc sounds like an a lot. Changing to a 500cc implant would likely require a lifting procedure. Changing the implant size is relatively easy. The breast lift can be complex. There are many different types of lifiting (mastopexy) procedures. It often comes down to how much skin needs to be repositioned and what technique your surgeon is most comfortable with. I would recommend discussing the different mastopexy techniques (wise-pattern, vertical, periaroelar, SPAIR) and the expected scar orientation with your surgeon.
