How to correct large eyelid space? (photo)

Question

Hi there, I am in my early 20's and have always had a large amount of eyelid space. The skin on my eyelid is wrinkly and is droopy. I'm not really interested in any fillers as they are temporary. I am concerned that a blepharoplasty would create an unnatural wide eye look - I have done some research on before and after pictures and have notices that a lot of times people end up with the "surprised eye" type of look. Any advice would help, thank you!

The deep upper eyelid sulcus [between eyelid and eyebrow] is actually a sign of aging, not youth. It happens as we lose sub-brow fat in aging, and as the eye socket fat moves from the upper orbit to the lower orbit due to gravity and loss of tissue support.In addition, droopy eyelids can decrease the soft tissue in the upper sulcus giving it a more hollow look. Raising the brow, in compensation can exaggerate the hollowness.I would recommend an in person consulation with an Oculoplastics surgeon to assess whether you have eyelid ptosis. Sometimes just raising the eyelid can help decrease the eyelid hollowing. I have posted a similar patient below in the web reference box.

Thank you for your question and photograph.  The deep space of your upper eyelid is typically considered a youthful appearance and I would think long and hard about considering changing it.  However there are techniques to do open fat grafting the upper eyelid space to lessen deep set eyelid fold.
