I am 200 pounds and I am brown skin I have very large breast in a DD bra size type I'm 5"1 and I'm looking to get a breast reduction done because of back problems but I am concerned about a raised bump on the bottom of my nipple. Im not sure if it's from sweating under breast or a swollen Montgomery gland. Could you please help me out.

Loose the weight you want first and then see how your breast look. If you have some lose skin or sagging, you can have a lift. A lift will also improve any asymmetry.
Your scars can be revised. During the healing from the reduction, swelling was placing some strain on your scars. With just the revision, there will be much less swelling and strain and your scars should heal better. Go back to your surgeon and discuss your options.
