Considering a breast reduction - should I be concerned about raised bump on nipple? 5'1, 200 lbs, DD cup.(photos)

I am 200 pounds and I am brown skin I have very large breast in a DD bra size type I'm 5"1 and I'm looking to get a breast reduction done because of back problems but I am concerned about a raised bump on the bottom of my nipple. Im not sure if it's from sweating under breast or a swollen Montgomery gland. Could you please help me out.

It is necessary to make a personalized evaluation and within the evaluation take into account several things: if it is hard, if it is soft, if milk comes out, if it has a different appearance. On the picture alone I can not give you an answer. There does not seem to be anything grossly concerning. 
I would recommend for you to reach out to a breast surgeon as that is normally their specialty!

The small bump appears to be a benign cyst. You would want to confirm this prior to the breast reduction surgery. I would suggest beginning with your primary care physician who can refer your to a dermatologist or surgeon if necessary.

Based on what you are describing, it does sound that you have large/heavy breasts causing physical symptoms, and breast reduction may be an option for you. 
It is difficult to tell what the "bump" is without further evaluation. I suggest that you seek an in person consultation with a Board-Certified Plastic Surgeon specializing in Breast Reduction surgeries to further discuss your options, specific techniques, concerns and expectations. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.

It is challenging to say what this is from the picturesyou included. Any new lesion on the breast would best be evaluated by a qualified provider, and I definitely would suggest this be the route that you take.
Any questionable skin lesion should be evaluated completely, possibly including a biopsy at your providers discretion, prior to considering elective breast reduction.
From the perspective of breast reduction, at 5 feet 1 inches with a double D cup breast, you would probably be a good candidate. The pictures that you included are not super helpful to evaluate the right and left breast from the perspective of their size. In general, good candidates for breast reduction have large pendulous breasts, with back and neck and shoulder pain as well as bra strap grooving. Many patients find themselves feeling awkward in social situations, challenges finding close that fit. These will be great reasons to seek the care of a qualified plastic surgeon in order to discuss breast reduction.

The technique I recommend is The Bellesoma Method. This will reduce and reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breasts is transferred to the underlying pectoralis major muscle resulting in pain relief without excessive reduction. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Unfortunately, with you being 5'1 and 200 lbs, your BMI is 37.5 which puts you at high risk for complications. You need to get your weight down to 170 lbs before undergoing surgery. The raised bump is probably a blocked Montgomery gland that can be excised at anytime.

That "bump" is neither, but rather a thicken scar, a hypertrophic scar or keloid. You may need some scar treatments post surgery to avoid more scarring like this.
