So during surgery, an experienced rhinoplasty surgeon can deproject and then reproject in the new position in order to maintain definition. If the nostrils widen, then a small wedge can be removed to narrow them.

Deprojection of the nose refers to decreasing the projection of the nose - how far forward the tip and / or dorsum appear. It is a very common problem that patients want improved. Depending upon how much the tip is reduced in height, the nostrils might start to flare. In these cases a small excision of the excess flare is performed. This is something that is routinely checked at the end of the surgery and is usually the last thing done.

Hello and thank you for your question. Nasal tip deprojection can be very effective for an elongated nose. The nasal tip will be swollen after surgery but this technique should not permanently make the tip wider. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. You should be able to view numerous pictures of actual patients with noses similar to yours performed by your surgeon. The answer is yes and it can make the tip look wider too. This is more likely in patients with thick skin. Some of these unwanted side effects of tip deprojection can be offset with techniques including skin wedge excisions of the skin at the base of the nostrils.
Hello, Deprojecting the nose will occasionally make the nostrils look wider, this is something that occasionally will need to be dealt with by narrowing the alae but not always. If there is no obvious alar flaring I will usually allow the swelling to go down and adjust the alae in the office a few months later if needed. As far as the tip, there are a number of tip maneuvers that can be performed to give the desired tip definition and these can be done whether you are deprojecting or not and should not be affected by deprojection. Yes, de-projection of a tension tip will certainly cause the nostrils to widen. How much depends on much the tip needs to be moved back. If the tip de-projection is anticipated to widen the nostrils excessively, an alar base excision may be planned at the same time. Discuss this with your surgeon. Good luck!
