The tissue is then dissected down to the gluteus maximus muscle, allowing for the buttocks to be lifted and reduced in size. One of the options, in your case, should be a combination of liposuction of hips and outside buttocks with augmentation/shaping of central buttocks with fat grafts. This should be done in, at least, two stages to avoid loose skin. Possibly some adjustment of gluteal crease as well. You need to find an expert to do this properly. good luck.
