The lateral canthoplasty does effectively lengthen the eyelid aperture laterally but the effect is subtle. Western canthoplasty should not be confused with what is claim for lateral canthoplasty as practiced in Asian countries, especially South Korea. In my opinion, these are disfiguring procedures that attempt to widen the eyelid fissure lateral by advancing the conjunctival surface.
For Asian eyes, the medial epicanthus can be removed in order to make more of the whites of your eyes show. Laterally, the current popular procedure is to make the outer corner less upturned - i.e., make them more horizontal, which will also increase the amount of show of the "whites" of the eyes. This can work well for the upturned eye; however, if the lateral canthus is already horizontal, then moving the outer corners further downward distorts the appearance of the eye and does not look at all natural.
