I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Thank you for your question. A large portion of my practice is dedicated to revision breast surgery. I have been in practice over 28 years. Your implants are on top of the muscle, and you have a typical "slide down" appearance. The best fix would be to place the implants back under the muscle and revise the mastopexy/lift as well is removing inferior pole breast tissue, so I can use a larger ultra high-profile implant. This a surgery which I pioneered termed reductive augmentation which creates a much more high and round appearing breast.. You should be able to have an excellent outcome. Best wishes to you.

You have large implants and large skin envelopes. Your breasts are hanging down inferiorly. To get the appearance you desire, I recommend taking out the implants and doing The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, implants can be placed if additional volume is desired.

Sorry to hear your not happy with your augmentation results. From the photo you provided it look like you could have benefit from a lift. I would also consider a change of plane, meaning putting the implants under your muscle with lower pole support (mesh). If you want a truly fake look, I would also consider increasing your profile to Ultra high and the most cohesive gummy bear.
You will unfortunately need another surgery, but go to multiple consults with surgeons and be very clear with what you are unhappy with.

If the first photo is you, you cannot have high perky breasts because your breasts naturally sit somewhat low on your chest and the bases cannot be moved. However, you can have a lift and raise the breast and nipples up some. This size is too large for you and will probably sag again if a lift is done. I would suggest going smaller or be content with what you have.

To make an accurate assessment of a plastic surgery outcome we always need to see proper before and after pictures.
If you don’t have before, and after pictures, ask your surgeon to forward the pictures they took.
If you’ve had more than one operation, then you should include before, and after pictures of each of the procedures.
I’m assuming the two pictures you’ve included represent the first outcome and the second picture is the outcome after the revision.
Breast augmentation outcomes is based on three variables.
The first is patient candidacy. The second is the choice of implants in regards to size shape and type.
The third is the surgeons ability to place the implants in the correct and atomic location.
Your initial patient candidacy was not consistent with your wishes and the look you wanted would not have been possible by simple augmentation alone.
Your second outcome is fairly consistent with an over augmented fake look in my opinion.
Using excessively large implants increases the chance of undesirable outcomes, complications And need for revision surgery.
Most plastic surgeons who do a lot of breast augmentation generally prefer placing implants under the pectoralis muscle.
It is a technically more difficult operation, but has many advantages.
If your second picture is a representation of your current outcome, then I suggest you stop where you are.
