I am a female, 46. Weightloss of 70 lbs. I had regained 25 back right before my surgery due to having to be off the tirzepitide shot before surgery. At the time of these photos, I'm 170 pounds. 2 previous c-sections. The procedure I had was a tummy tuck, liposuction ( contouring) and a hernia was fixed in the upper abdomen. I am unhappy with the placement of my scar and how it tapers down and puckers. Should I have had a lower body lift?

My recommendation for repair would be to perform a liposuction and scar revision of the two sides of the scar. Liposuction will help improve the contour differential due to the fat and the scar revisions will correct the unevenness and dog ears.

After 4-5 months from the liposuction, the lower abdominal skin incision can be revised. This way, the incision can be extended minimally while addressing the issue.

Liposuction of the hips/flanks and waist can further contour your curves. Further an exam is needed to know for sure but it doesn't appear a lower body lift is necessarily recommended. Abdominoplasty scars can be quite complex, ranging from the simple hypertrophic scar to a multiplane inflamed keloid. Pigmentation, discoloration, chronic inflammation, scar visibility can all occur in the short term.
