I had breast implants put in 10 years ago, Allegra Silicone 457, I am not certain on my pre op size, maybe 34A, but right now I believe I am 34D. I never got comfortable with the size and always felt they were too big for me. Lately I also started having neck and back issues. I'm planning on having kids soon, but I do not want to wait until after I'm done with all of them. Do you suggest i revise with smaller implants or explant for now? Do you think I will have enough tissue left if I explant?

Even with implants placed over the muscle, you still may have some animation distortion when you flex your muscles since scar tissue is attached to your muscles. However, for most women, placing the implants over the muscle helps hide some of this. It is not a "for sure" thing.
Your own tissue has sagged and can be relifted. Just based on your photos, your nipples are not too high and were placed in the proper location. The inferior incision can be extended to remove excess skin lateral to your breast. If you want to be smaller, more breast tissue can also be removed.
You might consider having the lump removed in case the scar tissue is causing some pain and also to be checked. the warmness could be from small sensory nerves that are around scar tissue. I doubt the weakness of your arm and shoulder are from your breast. You might want to ask your medical...
