What can be done for my alar retraction? 35yo, healthy. I've had a closed rhinoplasty 2016 then open rhinoplasty 2020 but the surgeon didn't fix the alar retraction from the initial closed surgery. (I did ask for it, he said he might do a graft when I am in surgery, but when I woke up he said it wasn't needed and as it heals it will improve but it's been like 3 years and looks the same...) Should I get an alar composite graft under local? Or fillers? Any and all suggestions welcome, Thank you

Just holding your nose each day won't help. Rhinoplasty can be done to take down that bump permanently, making it look better, but natural as well. At the same time the septum can be straightened so that your breathing gets better as well. Be sure to see a rhinoplasty specialist for consultation.
It's always best to fix outside and inside problems at the same time with the nose. We take insurance for the inside, but the rhinoplasty for the outside is considered cosmetic. Be sure to see a rhinoplasty specialist for consultation.
While it's hard to give a full evaluation without full side views, from your photos, you are a good candidate for rhinoplasty. The tip can be made thinner and lifted up to be shorter. The bumps can be smoothed out as well. Be sure to see a rhinoplasty specialist for consultation.
