I would recommend a combination of surgical procedures to achieve optimal results. You appear to be a good candidate for a deep neck liposuction, buccal fat reduction, and a possible lower face and neck lift. The buccal fat reduction would aid in slimming your face overall. The liposuction would address the stubborn fat in the submental area but not the excess skin, whereas, the lower face and neck lift would address the sagging skin around your neck, jawline, and jowls. Notably, the procedures I have mentioned may change based on a multitude of factors such as your age, preexisting medical conditions, etc. Going forward, I would recommend you to complete multiple in office consultations with board certified facial plastic surgeons to discuss your ideal outcome. The limited photographs demonstrated fat deposits above and below the platysma muscle in the neck which will require a neck lift procedure. You also appear to have loose and inelastic skin in the neck,and if that's the case, then a lower face and neck lift is required to tighten that loose facial and neck skin. Simply removing fat in the neck will make the skin issue worse

Answer: How to get the best results If you want the most impressive results than a neck lift a.k.a. facelift is a far better procedure. Liposuction simply reduces fat and fat distribution is not really causing that much problem for your neck in the first place. Removing fat can help and some people significantly. This is especially true for young people who are slightly overweight. As patients get older other issues like the underlying platysma muscle and the lack of skin elasticities become much bigger problems and should be treated to get the best result. Typically around the age of 50 is when patients stop being reasonably good candidates for just liposuction. In the end it depends what you’re trying to achieve and what you’re willing to go through. If you want a very subtle improvement in Liposuction may do just that. If you want to well-defined neck and jaw line than a neck lift will do a much better job at it. As always I recommend multiple in person consultations with local board-certified plastic surgeons. During consultations ask providers to show all of their before and after pictures for the procedure you’re interested in. Experienced providers should have no difficulty showing 50 or more sets of before and after pictures. Avoid providers with any significant number of justified negative reviews on various physician review websites.
