Performing osteotomies (the hammer that is used in surgery to fracture the bone in a controlled fashion) is one of the techniques to narrow the bony portion of the nose. As far as the lower two thirds of the nose, cartilaginous techniques to narrow and refine the midvault, tip and nostril are often needed to accompany the former and thus achieve more defined and balanced aesthetics overall.
Great question! The upper portion of the bridge is composed of bone. The only way to narrow this is to fracture the bones on both sides and bring them in to narrow the bridge. The tip of the nose is made of cartilage and the excess cartilage can be excised to create a slimmer appearance. Your surgeon canaccomplish this by trimming, suturing, and reshaping the cartilage in yourlower nose. You may also benefit from an alar base reduction. Osteotomies may also be beneficial. Makesure you specifically look at before and after pictures of real patientswho have had this surgery performed by your surgeon and not just a computeranimation system. The most important aspect is to find asurgeon you are comfortable with. Osteotomies are a very routine component of rhinoplasty, and are performed in the high majority of cases to narrow the entire bridge line. There also mandatory after any dorsal hump removal because the hump removal itself creates an open roof, flattop nasal deformity. Osteotomies accomplish narrowing the upper 2/3's of the nose which include the nasal bones and the upper lateral cartilages which is the also known as the bridge line. A tip plasty accomplishes narrowing the nasal tip cartilages. An alar-plasty accomplishes narrowing wide nostrils at the base of the nose. Every nose is different and unique to that individual. If the appearance of your nose concerns you enough to do something about it, it can be improved with a natural artistically performed rhinoplasty. What would be the best option and exact procedure would depend on an in person examination (not just an online consultation) and discussion about desired outcome. Excellent results can be achieved with either a closed or open technique. Keep in mind that following advice from a surgeon on this or any other website who proposes to tell you exactly what to do and how technically to do it based on two dimensional photos without examining you, physically feeling your tissue, assessing your desired outcome, taking a full medical history and discussing the pros and cons of the operative procedure would not be in your best interest. Natural appearing results need to be individualized and what is appropriate for one patient is not necessarily the best for someone else. I would suggest that you find a surgeon with extensive rhinoplasty experience certified by the American Board of Plastic Surgery and one who is ideally a member of The American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with. Once the desired result is agreed upon the surgeon will discuss the techniques which will be used to attain your desired result. In your case you will need osteotomies, nasal tip refinement and reduction of the alar base. You will need several things done to narrow the nose. Your osteotomies (bone breaking) will need a special technique to only move the lower part of the bones inward. The top part is already narrow. In addition the tip cartilages and the alar (nostril "wings") need to be moved in. Seek out a consultation with a board-certified rhinoplasty expert. Best wishes.
