What this means in terms of your tip definition is that simply removing excess tissue will not provide significant refinement or reduction in the bulbosity of your nose. Similar to trying to see what is underneath a thick duvet cover, in order to see the shape of your tip underneath your skin, the tip needs to be projected, and made a bit taller in order to show through your thick skin. Building up your bridge will also make your nose appear smaller and slimmer from the front.

Answer: Thick skinned noses. You indeed have a very thick skinned nose and that is part of your ethnicity. It is also a short nose. My advice to you is to leave it alone based on 35 years of rhinoplasty and revision rhinoplasty. There is too much risk of making it worse. I do a large number of revisions from patients who say "I wish I had my old nose back".

Answer: Asian rhinoplasty with thick skin you may require some cartilage grafting to the tip to create a sharp reimage as well as very subtle augmentation of the dorsum to build abridge up and create a nice contour that would blended in with your new tip. Answer: Nose surgery Sparkles2000, I would most likely recommend a combination of a rhinoplasty followed by alar rim surgery. I prefer not to alter the shape of the nostril at the time of the original rhinoplasty. I wait until the tip has settled and presented itself to the point that I feel comfortable taking this step. Why do I stage this? First of all it is not reversible so if one takes this step it must be done as precisely as possible. Secondly, when I change the projection, orientation and shape of the nasal tip it will change the shape of the nostril in a way that is unpredictable which harkens back to my first point. Finally it is a patient friendly step, can easily be done under straight local, with minimal bruising, swelling and post op discomfort. See 2-3 facial plastic specialists and discuss your expectations. Good luck!
