Lower Lid Retraction and Entropion (Photo)

Sadly these types of issues can occur. Well meaning efforts to prevent the issue or manage it conservatively with massage, taping, and injections are often insufficient. Many surgeons who are in the repair business try to shorten the lower eyelid further in an attempt to fix the problem. That does not work. Some may offer a skin graft into the lower eyelid. These type of repair is almost as disfiguring and the problem itself. The answer is a cosmetic repair that recruits more skin and soft tissue into the compromised lower eyelid. My approach (graphic surgical video attached below) is to vertically lift the cheek on a custom made orbital rim implant of ePTFE. The ePTFE implant material is FDA approved for deep facial implantation. This implant serves as a felting material to hold the lifted cheek bringing needed skin and soft tissue into the eyelid. A second implant of hard palate graft borrowed from the roof of the mouth is used behind the eyelid for the purposes of shaping the eyelid. Over the course of 25 years I have done thousands of these repairs. However there is no substitute for a detailed in person consultation to fully assess your current situation and determine precisely what you need.

There may be some scarring on the conjunctival surface, which might require revision surgery. Following surgery 5-FU injections may needed.
