People who get a double chin do not have an abnormal fat distribution. This condition is always associated with a facial, skeletal imbalance with the mandible being small in the relationship with the maxilla. It’s not the chin itself but rather the body of the mandible that is related with the development of a double chin. People who develop a double chin, almost universally have an overbite dental occlusion. This means on profile pictures. The upper lip will have more forward projection than the lower lip. Advancing the mandible with formal jaw surgery is more than most people are willing to undergo. It is not a pleasant procedure to recover from. Subcutaneous is the problem but is the most common treatment option. Liposuction can give good improvements and is most likely the only realistic option other than a neck lift which is a better option in older individuals. Up to about the age of 40 Liposuction is a viable option and once people get to the age of 50 and neck lift a.k.a. facelift is probably a better treatment option. The biggest issue with moving forward with Chin and neck Liposuction is that there is a substantial degree of variation in skill for this procedure among providers. Being board-certified and plastic surgery with year of experience is not sufficient to confirm someone has approach mastery for this kind of work. If it’s not done correctly, the first time Then it’s very difficult to ever get a good outcome. Finding sufficiently experienced and talented plastic surgeon is much more difficult than most people realize. It generally means having multiple in person consultation while carefully scrutinizing lots and lots of before and after pictures. Bring pictures of yourself taken the plastic surgeons take before and after pictures. Have someone else take the picture or use the timer in your camera. Don’t take pictures using mirrors. We need a quality, frontal picture and quality profile picture taken at correct angles. Bring those pictures with you to use as reference during the consultation. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar facial characteristics. In the hands of the right provider you should have access to see more before and after pictures then you have time to look at during the consultation. Being shown only a handful of examples might be a major red flag that the provider doesn’t have sufficient experience. Highly experienced surgeons should have hundreds of before and after pictures to choose from. Before being able to vet providers properly, you need to get a good understanding of what quality results look like and begin to understand the patient candidacy and how this impacts the potential outcome. Overall, it’s pretty complex and overall I would say patience are not very good at selecting plastic surgeons. There isn’t much you can do regarding your candidacy for surgery so the only variable you have is provider selection period. There is really no treatment other than Liposuction that is going to give you the potential of a decent outcome. Understand that Liposuction isn’t treating the underlying primary problem and anytime we select treatment options that don’t address the main problem. Partial improvements are the best you can expect. I don’t think non-surgical treatments are a good choice for this. Best, Mats Hagstrom MD.
