Tethering after breast explant en bloc. What other option do I have other than surgical? (photo)

Unfortunately, I'm not aware of any effective non-surgical solutions to fix this problem. Options such as fillers will only temporarily improve things. Again, with dubious results.

You did not state how far you are out from your operation. It's possible some or all of the contour issues are due to swelling. It looks as though if the crease at the bottom of the areola persists after healing is complete it could be revised under local anesthesia. From the photographs it does not appear that a mastopexy is indicated.

Thank you for your question and for your photo. It can take up to 6 months to fully heal. I would advise you to massage the area to improve the tethering. Hope this information helps.

Thank you for sharing your question and photograph.  You did not mention how long it has been since your en bloc but I would first allow a full 6 months to pass while performing aggressive scar massage and tissue massage techniques.  Based on severity of your tethering and scar indentations you will most likely require additional surgical intervention but be fully healed before doing so. 

Only on the basis of in person evaluation one can give you a reasonable advise. Options given by your surgeon seem to be very reasonable. Considering that it is limited area and requires relatively small volume of fat grafts one may consider combination of twilight/local anesthesia? Second opinion consultation should be a consideration. Good luck.

To most accurately answer your question would require an examination . However, based on your photo alone , some type of surgical correction will be required. This might involve scar revision , with release of the scar and fat grafting, or possibly a mastopexy. Regardless of what procedure would be required , it should be able to be accomplished under local anesthesia with IV sedation, or local alone if avoiding general anesthesia is a concern.

if you had your surgery recently, then there is a chance that this will improve with time. Also, if you plan only a minor revision, local anesthesia might be an option. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

You have some scar implosion or inversion which can happen particularly after a periareolar approach. You will be likely need a scar revision where the deeper layers are freed up and reclosed +/- fat grafting.  

The length of time from your surgery will play a big role in what your current options are. If you are still early post op, massage can help break up that scar tissue and improve the tethering.
