I have this dimpling around my mouth. Not sure if it is marionette lines or pre jowls. Do I need filler or something else?
Answer: Dimpling around the mouth Thanks for your question. In looking at your picture it is evident that there is volume loss around the mouth causing the dimpling and depression below the corners of the mouth and onto the chin. These areas could be filled out and balanced to blend with the mid face giving you a better more youthful look. Some filler over the lower jaw could even out the jaw line. I could not tell from the picture about any skin problems but that could be addressed also. Hope this helps Dr. ODell

Answer: Drooping of skin around the Marionette lines You are experiencing some drooping of the skin around the mouth and it is causing the skin t0 bunch around the mouth. This is caused by the fact you are losing some volume in the cheeks and in the area below the cheeks but above the corners. I would recommend a filler (Lyft) in the cheeks to help lift the skin. Also, I would use Sculptra to volumize the submalar areas (oblique to the corners of the mouth) and likely the preauricular areas(near and in front of the ear area) to lift the lower 1/3 face. Lastly, you will need a filler (Juvederm) in the Marionette lines and next/near the chin area.

Based on your posted photo, I would recommend an initial resurfacing procedure such as fractional CO2 laser resurfacing or skin needling to induce collagen production and produce a smoother surface. This should be followed by a review 3-4 weeks later to assess whether a fine amount of dermal filler would be suitable to touch up the deeper and more stubborn pockets of indentation/ unevenness.
