Hello, I had surgery 5 months ago and asked for 235-250cc low-moderate profile implants to give a natural and soft look. The surgeon suggested round silicone under muscle at 325cc, high profile. I am 5’7 and little body fat with quite a bony chest wall. I cried and after shouting at me that he was the surgeon and he knew best, he agreed to 280cc. They are so fake looking and rock solid. They don’t move easily and sit in my armpit so get quite sore. What options do I have to correct them?
Answer: Revision surgery Dear Nurturing721107, breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals. Answer: Breasts Your breasts naturally sit wide apart and the bases cannot be moved. With little breast tissue, you are feeling the implants. If they are placed on top of the muscle, they will look more fake and the edge might show. You could change to implants in front of the muscle and textured and anatomical shaped, but the improvement may be small. Otherwise, your lateral pockets could be tightened some. You don't appear to have donor fat. You could also go down in size and have more room for your implants. Or, keep what you have. Your anatomy does not give a lot of options.

Answer: Breast Implant Revision If you are capsulated, then you will need a capulectomy or neo-pectoral placement of the implants. Also I would recommend wider and flatter implants to give you more clevage (closer together) and to get rid of the fake look and make them more natural looking. I would definitely stick with silicone implants, as you are very thin and silicone will help minimize any implant palpability/rippling. Hope that helps.

Answer: Implants very wide apart and fixed in position. What options do I have to correct them? Your implants are under the muscle .The muscle is pushing them apart. I would recommend placing the implants above the muscle .

Answer: Decisions regarding breast augmentation I generally encourage people not to travel long distances for elective surgical procedures, and definitely not travel abroad for cosmetic surgery. I generally encourage people to multiple in person, consultations before choosing a provider and scheduling surgery. Ultimately, your surgeon is responsible for the outcome of the procedure while you are responsible for choosing that surgeon. Patients need to have a good understanding regarding their own candidacy for a specific procedure to have a good idea of what results are likely to look like. Your breast are divergent and sit far apart on your chest wall. Augmenting the breast will emphasize the divergence and make it look more apparent. The bigger the implants you choose the more the divergence will be obvious. Considering the distance between your breast, a low profile, wider diameter implant may have been a better choice. It also looks like your implants are bottoming out giving too much fullness in the lower half of your breast. If your implants feel hard, then you may be developing a capsular contraction. Either follow up with your surgeon or schedule in person second opinion, consultations with other plastic surgeons in your community. For second opinion consultations patient should ideally come prepared bringing with them a complete set of before and after pictures, a copy of your operative report ideally a copy of your consultation note.
