You should wear a surgical garment or sports bra for 6 weeks post surgery. You should go for regular check ups to ensure you are healing well and your surgeon will be able to tell you when it is safe not to wear a bra at night.

This is to give time for the implants to adhere to the surrounding tissues. It also provides support and allow for a smoother and less painful recovery period.
Hello and thank you for your question! When you are cleared to stop wearing your compression bra, you should be able to sleep without a bra as soon as you would like. Sleeping without a bra after your implants have healed should not be a problem. However, it is important that you wear your post-surgery compression garment at all times until your plastic surgeon has advised you otherwise -- supporting your new breasts and inframammary folds, especially if your implants were placed under the muscle through an inframammary incision. Depending on your surgeon and a few other individual factors (such as your implant type and size), this period can be anywhere from four to six weeks. You should confirm with your surgeon or his/her staff how long they recommend use of a bra both during the day and while sleeping. It is also important to discuss with your surgeon their preference regarding when you can begin wearing a bra with an underwire- plastic surgeons have different opinions regarding this topic. This varies between physicians,but we allow our patients to discontinue the bra at night at 1-2 wks. I would recommend that you contact your surgeon's office and ask them and follow their instructions.
Opinions will vary by surgeon on when you can sleep without a bra after surgery. I recommend speaking with your surgeon as he/she is most familiar with the details of your surgery and will be the best one to advise you.
It really depends on your surgeons preference, but probably equally important is if where your implants were placed. If they are in front of the muscle, it is important to have continual bra support until the capsule that your body creates around the implant is well formed = a couple of months. If the implant is behind the muscle, and the origin of the pectoralis muscle on the ribs was not released or only partially released, you can go without a bra much sooner. If the muscle was released, once again it is important to wear a bra until the capsule is well formed.
