Question

I had a lift and 320cc over the muscle implants put in 2 years ago. I’m not happy with how bulky and heavy they feel, and the level of lift is not as much as I’d like to see. For those reasons, as well as others, I’m exploring maybe explanting altogether. I’m worried that I’ll be left with similar level of sagginess without getting another lift. Because I’ve already had one, I don’t know if I need another one. Is a secondary lift recommended?

Without examining you or seeing photos its difficult to give specific advice. Some patients need a lift after explant, many don't. Its reasonable to take the implants out and wait a year to see how the breasts contract before committing to a lift. The only way to know for sure is to book a consultation with a board-certified plastic surgeon near you to see what your options are. Best of luck!

If you are already concerned that your lift isn't adequate, it is not going to improve with removal of the implants.  It is likely that you'll need another lift to achieve your goals.  I encourage you to meet with a board certified plastic surgeon in your area for a consultation.  Together you can determine the plan that is best for you.

Usually removing them and performing the lift at the same time makes the most sense.  If the implants are removed, there will likely be flattening and drooping that can be corrected in large part with a breast lift.   As far as the volume that is attained, that is a bit difficult to predict given the time of 2 years that you have had large implants thinning the remaining tissue.  Removal followed by a waiting period could also be considered. Find an expert at all aspects of breast surgery and set up an in office consultation.Kenneth Hughes, MD, Board Certified Plastic SurgeonLos Angeles, CA

technique I recommend is explantation using The Bellesoma Method.  The implants are removed, your breast tissue
is reshaped creating upper pole fullness, elevated higher on the chest wall and
more medial to increase your cleavage. 
Vertical scars are avoided, nipple sensation and the ability to breast
feed are maintained, if not injured during your previous surgery.  At the same time or later, fat transfers can
