If I get an explant, will I need another lift? I had a lift and 320cc over the muscle implants put in 2 years ago.

Without examining you or seeing photos its difficult to give specific advice. Some patients need a lift after explant, many don't. Its reasonable to take the implants out and wait a year to see how the breasts contract before committing to a lift. The only way to know for sure is to book a consultation with a board-certified plastic surgeon near you to see what your options are. Best of luck! 

Usually removing them and performing the lift at the same time makes the most sense.  If the implants are removed, there will likely be flattening and drooping that can be corrected in large part with a breast lift.   As far as the volume that is attained, that is a bit difficult to predict given the time of 2 years that you have had large implants thinning the remaining tissue.  Removal followed by a waiting period could also be considered. Find an expert at all aspects of breast surgery and set up an in office consultation.Kenneth Hughes, MD, Board Certified Plastic SurgeonLos Angeles, CA

technique I recommend is explantation using The Bellesoma Method.  The implants are removed, your breast tissue
is reshaped creating upper pole fullness, elevated higher on the chest wall and
more medial to increase your cleavage. 
Vertical scars are avoided, nipple sensation and the ability to breast
feed are maintained, if not injured during your previous surgery.  At the same time or later, fat transfers can

If you are already concerned that your lift isn't adequate, it is not going to improve with removal of the implants.  It is likely that you'll need another lift to achieve your goals.  I encourage you to meet with a board certified plastic surgeon in your area for a consultation.  Together you can determine the plan that is best for you.

Thank you for the question.  I suggest that you submit  photographs, side and frontal views, taken by a second party with your arms by your side. Generally speaking, what your breasts will look like after explantation depends on several factors such as: the quality of skin elasticity (the better the elasticity the better the skin will bounce back), the size of the implants used (the larger the implant the more trouble you may have with redundant skin), and the amount of breast tissue present at this time (which may have changed since the time of your breast augmentation).
Life experience since your breast augmentation procedure, such as pregnancy or weight gain weight loss, will potentially influence the factors discussed above. If you take these factors into consideration and apply them to your specific circumstances you may get a good idea of what to expect after the implants are removed.
 In "borderline" cases where there is doubt whether breast lifting is necessary or not, I advise patients to undergo explantation only and to give their breasts the benefit of the doubt  before deciding whether breast lifting would be beneficial.  Patience is definitely indicated after removal of breast implants; it will take many months/year for the skin to “bounce back” and for you to see the final appearance of the breasts.
In your case specifically my best advice: select your plastic surgeon very carefully. Ask him/her to show you examples of previous patients (demonstrating what he/she expects your breast to look like after breast implant removal and breast lifting surgery.   Seeing these visual representations (of similar patients), will likely help you make an informed decision as to how to proceed.

Since, without photos, we have no knowledge of your anatomy it would be impossible to give you any thoughts pertaining to your anatomy. My recommendation would be to see 2-3 experienced consultants for an exam and get their thoughts. I will say for most patients I see with your concerns we explant and wait 6-12 weeks to see what the breasts are really going to look like. Half the time the patients decide to forego any  lift.
