I currently have 275 mentor gel implants. Originally I was a size 34C however over time with a little weight gain and menopause they are now a DD and they come out the sides of my bra and under my armpits. I wear sports bra’s all the time to smoosh them down. Remove implants and do a lift which would make them alot smaller and less full, switch out for smaller implants and revise pockets, or keep current implants and remove some breast tissue especially laterally and correct pocket.

Answer: Originally 34C now 36DD, options to making smaller? Thank you for the question. Based on your question, I think you may already have a very good understanding of what might be involved with downsizing of breast implants. As you mentioned, adjustment of the breast implant pockets (capsulorrhaphy) will likely be necessary to achieve the outcome you are looking for. Depending on your physical examination, additional "maneuvers" such as breast lifting may be necessary. Generally speaking, the best online advice I can give to ladies who are considering revisionary (downsizing) breast augmentation surgery is: 1. Concentrate on choosing your plastic surgeon carefully. I think that this will be the most important decision you make. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or “down a cup size” or "C cup" etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. 3. Once you feel you have communicated your goals clearly, allow your chosen plastic surgeon to work with you in determining the best plan to achieve your goals. I hope this, and the attached link (dedicated to downsizing breast implants surgery concerns), helps. Best wishes.

Answer: Options for making implants smaller Your situation is very common, as I see it often in my office. More and more women are choosing to have their implants removed all together once they hit menopause and their breasts naturally get bigger. I would recommend an explantation, pocket tightening (capsulorraphy) and a lift to reshape the breast and get rid of excess skin that would cause sagging. Seek a consultation with a Board Certified Plastic Surgeon who performs a lot of explantation surgeries. Best of luck to you!
