It is impossible to be precise without a consultation in person, but looking at your picture from the front you seem to have II degree breast ptosis, and a significant asymmetry, with one breast lower than the other. To achieve a good correction of the asymmetry and of the ptosis, I would personally suggest an uplift.

In my experience, placing a single implant on one breast would never give a nice outcome. The shape and fullness of the breast with the implant would be, in fact, completely different from the other side.
You would need a combination of techniques to correct both the differences in volume and in skin envelope. Generally this is achieved with implants (of different volume) and an uplift on the breast with the bigger skin envelope. A simple uplift/reduction on the bigger breast may give some improvement but, in my opinion, it would still offer an inferior result compared to the combination with implants.

It really depends on the preferences of your surgeon, on the position of your original scar, and on your specific anatomical characteristics (how large your areolas are). The correction can be performed successfully both from an inframammary and periareolar approach.

Correction of tubular breasts normally involves breast implants (either subglandular or submuscular, depending on the preference of your surgeon), internal release of the tissues, periareolar mastopexy. Looking at the pictures that you have posted, it would appear that you had some breast ptosis to begin with, that could benefited from an uplift. Your nipples appear lower than the submammary crease, even in pictures taken with your arms elevated, so if additional scars are not an issue you may want to discuss an uplift with your surgeon.

ive always had what I would describe as "big boobs" but at 24 years old and after two children they went down from a 32E to a small C. And dropped. ALOT. I had my first consultation with Mr Adamo at the hospital group. He advised me that I would need a lift, with 385cc gel implants, high profile and over the muscle to achieve the look I want. ( which was a DD, a bit big but natural and the ability to wear a vest top without having to worry about a bra to hold them up! ) He also told me he could make my aereolas smaller which is something I've always wanted even before my kids. I didn't even bother having another consultation else where and booked my op, as I did my research and two of my friends went with the hospital group and couldn't recommend them enough. On the day of surgery Mr Adamo measured me again very thoroughly and was very honest with everything. He told me that he likes to only have a few patients as it means he can take his time and doesn't feel rushed which really put me at ease ! I couldn't of been made to feel more relaxed by everyone there. The whole experience was amazing and I Couldn't fault a thing. 3 days post op and I'm over the moon! Still in some pain as I expected but whenever it hurts I just look at my new boobs and it's so worth it! The first photo is starlight after surgery and the one below is 3 days post op.
