They haven’t always been like this and it is a semi recent change. I was wondering if anyone could tell me why this would happen and if I should be worried. This has also been accompanied by tenderness and a lump under both of the nipples. I am on no medication.

The permanent stitch can be released. It is unlikely that at 3 years post op the scar will widen. You can then undergo a repeat donut mastopexy. However, you have asymmetrical nipple-areola complexes. You would benefit from revising both nipple-areola complexes.

It appears that you have inverted nipple. Usually reason for that are short and dysfunctional milk ducts. Surgical correction may help. In person consultation with a plastic surgeon with experience in this problem is recommended. Good luck.
