As we age, our eyelids naturally sag and droop. A combination approach would be best to see visible improvement. In terms of non-surgical treatment options, I would recommend a Chemical Brow Lift (using Botox, Dysport, Xeomin, or Jeuveau) and filler injections to rejuvenate the temples, eyebrow, eyelid, eye troughs, upper eye hollows, and forehead. I typically combine this with Ultherapy and thread lifting to lift the brow; RF and skin tightening laser treatments to stimulate collagen production and rejuvenate the skin; and laser or superficial RF devices for improvements in texture, color, and any crepey skin around the eyes as needed. Thank you for your question. First, and evaluation to assess for the reason behind the droop must be performed to make sure there isn't an underlying medical reason for it. If not, then there are a few surgical approaches to tightening the upper eyelid to make it drop less. In many cases, this can be performed using an incision under the eyelid. Hello @Sammyb56 Asking for advice on plastic surgery can be tricky. If you want honest feedback, it's important to use pictures that accurately represent what you're looking for. Blurry or otherwise poor quality photos does not help much. That said, you could benefit with a brow pexy or Browlift form what I can see in the picture. The most appropriate course of action is to consult one or several board certified plastic surgeons. This will allow you to get more information and options tailored specifically to your needs. You likely need left upper eyelid ptosis surgery. See an oculoplastic surgeon for evaluation and treatment.
Eyelid surgeons love posterior approach ptosis surgery (Müllerectomy) because it take them 15 minutes to perform. I mention this because it is often the wrong type of ptosis surgery. In my opinion, it will be the wrong type of surgery for you. I suspect you need an anterior levator resection ptosis repair. You need a detailed personal consultation. Do not be impressed by the neosynephrine test. It is not accurately predictive of your response to ptosis surgery. You also need to be assessed for the need for ptosis surgery on the right side as well. Your eyes can be your best feature.
One option is an upper eyelid blepharoplasty with levator plication. This can be done under local anesthesia to assess the amount of correction at the time of the procedure.
You appear to have ptosis of your upper eyelid. This condition occurs when a muscle in the upper eyelid becomes weakened and it allows your eyelid to fall lower across the eyeball than it should. This problem can be corrected with a surgical procedure. I would consult with an oculoplastic surgeon or a surgeon experience in ptosis repair.
