I currently have 500cc saline implants that are under the muscle and I am scheduled to have them exchanged for 700cc silicone implants this month. I want to go bigger and my surgeon has assured me that size “will be fine” but now I am reading all kinds of horror stories about implants that big. My worst fear being the dreaded “uni-boob”. I am 5’7 and 135 lbs. Does this size seem appropriate or am I begging for complications?
Answer: 700cc implants There are multiple reasons not to use 700cc prostheses. First, the 500cc implants have already "bottomed out" and 200cc more would stretch the tissue more. Second, the implants need more support and I would recommend placing Alloderm/Strattice as a sling acting as an internal bra at the lower aspect of the breast. This will give you the internal support needed for the implants. Intraoperative, different sizers can be placed to see which gives you the ideal shape and size. By tightening the pocket, the "uni-boob" can be prevented and the ideal implant size can be determined.

You need a breast lift, not larger implants. The technique I recommend is explantation using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation and the ability to breast feed are maintained, if not injured during your previous surgery. Keep in mind that sometimes adjustments of the surrounding breast implant capsules (e.g.capsulorraphy) is also necessary to achieve the desired outcome. You will find that online consultants will not be able to provide you with specific advice in regards to selection of specific breast implant size/profile. Ultimately, careful communication of your goals will be critical. Generally speaking, the best online advice I can give to ladies who are considering revisionary (upsizing) breast augmentation surgery (regarding breast implant size/profile selection) is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "D or DD cup” or "more full and plump look" etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. 3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Answer: Breast revision Hello and thank you for your question. It appears that you have room for bigger implants than what you have. I am not sure what profile your current implants are, but if you change to a higher profile implant with slightly larger base width, you will probably be close to 700cc's. My main concern is that your breasts look very bottomed out as your distance from your nipple to your fold seems excessive. If you were my patient, I would upgrade you to larger implants and perform a smile mastopexy (where a crescent of tissue is removed from your inframammary fold) to give you the ultimate shape and a well-hidden scar within your crease. The point is that an experienced aesthetic breast surgeon will be comfortable with any sized implants when the pocket is made to securely fit your implant; the smile mastopexy may be necessary to ensure that your nipples are centered over the most projecting parts of your implants.
