Peau d'orange is caused by swelling near the skin surface usually from trapped or slowly draining lymphatic fluid among other things. See your surgeon to make sure you don't require and active fluid drainage.

Thanks for the question and the photos. The surgeons that have answered have given nice responses as to what is going on.  When we have seen this, in most every instance this has resolved.  What we do now is that post-SmartLipo, we will use one of the advanced RF skin tightening devices to contour the skin more and at the same time resolving this concern in almost every case.
