I am 38 years old and otherwise feel like I look fairly young. These two bulges have started to form around age 35 and have just been getting worse. I was told I could do filler around the areas to 'fill it in' but I'm not sure the best solution is to add more volume to the lower half of my face? I also consulted a plastic surgeon who told me he could lipo them but that seemed like a bad idea? Or maybe that is my best option? Do I have a week chin?

Answer: Fillers and Kybella Hi there! Thank you for your question. You have a valid concern, as this fullness on each side of your mouth can give you a frowning or jowling appearance over time. It looks like your face is otherwise on the thinner side, which can accentuate these areas of concern. I suggest the following: 1. A little filler to the cheeks to help pull up and create balance between your upper and lower face. 2. Filler to the area just below the "bulges", on your chin. See how it looks like someone pinched your chin and left their fingerprints on each side? These depressions are creating a shadow, which again, accentuate the bulges. 3. Small amounts of Kybella can be injected in this area to help dissolve a little of the fat. I would not recommend lipo suction for this particular area. It is too delicate, and lipo suction, if done too aggressively, can create permanent, unsightly divots or depressions on your face. Try the filler first. It is not permanent and will help camouflage the area. If this does not alleviate it to your satisfaction, try the Kybella. I hope this helps! Ivette Ballara, RN

Answer: Yes. I do believe filler placed properly can camouflage this to some extent Sometimes there is a bit of fat herniation between muscle (here between the DLI and DAO), unless you have preciously has filler and it is old filler product. We can often camouflage this issue with very precisely placed filler. Not a lot. Placement matters. Product matters. I would needle place (rather than cannula) — as just trying to decrease the step-off of prominence compared to the adjacent skin. I would use Restylane Defyne as it has XPreshan technology, that tends to integrate into the dermis more than aggregate in a clump.
