I had a scar revision done 3 years ago from a past accident. The dermatologist performed a sort of "Z plasty" and the resulting scar left a straight line that is indented, which is what bothers me because of the way it catche slight and casts a shadow making the line look more prominent. It's somewhat red in person and a little tender at times. Would injecting filler or some implant under the scar help raise the skin surface and or lessen the indented appearance? Other options?

Unfortunately filler and implants are not the solution., Those would only add volume to the area, and not in the way you are looking for. I believe your best option is a scar revision. Although the scar cannot be entirely removed, it can be made to appear less so. With a revision, the scar can be transformed into more of a line, rather than the "z" appearance it currently has. The scar will be parallel to the philtrum (the vertical groove between the base of the nose and the border of the upper lip) which will flow better with the natural features of your face. This will also improve the way lighting/shadowing hits it, reducing that "prominent" appearance.
