The fat necrosis needs to be removed first and then secondary fat grafting to improve overall contours can be performed.

Fat necrosis from a previous surgery does not prevent revision surgery.  You may want to consider having a slightly lower volume of fat injected.  Also, the surgeon will need to be very precise in placing the fat to optimize its survival.  For that reason, the procedure could take longer than average.  I would wait until the scar has completely healed after they excise the previous necrotic area so that your surgeon will know what to do to address any deformity that results from its excision.  Good luck.
