As far as I know coolsculpting is purely cosmetic and is not covered by insurance. The procedure is performed to improve appearance by reducing fat content.

If you found an insurance provider that will cover CoolSculpting then you can call the insurance company and ask them how to bill accordingly, but I have not heard of any insurance provider who covers CoolSculpting.
