Hello RSM Jay in Akron. Thank you for your question regarding options to improve uneven gynecomastia results and skin creases.Thank you for your pictures and your history. In analyzing your picture, you have excess breast skin. The only way to get rid of excess breast skin is by removing and tightening the skin. This can be achieved by making an incision from your armpit down the side of your chest. In this procedure, an elliptical area of skin and fat is removed and the lateral breast skin is pulled to the side. This will eliminate your excess breast skin. Alternatively, an incision can be made underneath your breast where the breast meets the upper abdominal wall, but that incision would be highly visible.Another option is to consider pectoralis implants. A pectoral implant will fill out the chest and make it appear that you are extremely muscular. Considering all factors, I think pectoralis implants would be your best option. All these gynecomastia correction procedures can be done under general anesthesia in an outpatient setting.Good luck. Thank you for questioning your pictures. This is a tough situation. Obviously you have an excess skin that did not distribute evenly. I have two thoughts on the matter: number one – you can have revision liposuction with the addition of body, tight or vision visor liposuction. In hopes to help skin retraction. Number two – excision of excess skin, so that the scar is at your natural crease. Removal of your natural nipple and replacement in a more aesthetic place after skin removal at the same operation. Skin creases after liposuction procedures can be a result of loose skin, scarring or usually both. 4 months is still early on in the recovery. If they are still present at 1 year after the surgery, options include scar release with fat grafting or skin excision if there is significant loose skin.
It is too early to evaluate your result. Skin may improve and contract further. Irregularities and lumpiness after liposuction/ excision will also improve in coming months. Some patients who had very large, feminine breast may need second stage surgery to remove extra skin and shape the breast. In that time all irregularities and bulk may be addressed. You should wait at least 6-8 months before considering revision.
To make an assessment regarding the outcome of any plastic surgery procedure with generally always need to see a complete set of proper before and after pictures.If you don’t have before and after pictures and ask your surgeon to forward the pictures they took.There are three variables that contribute to fullness in the Male chest. These are skin laxity or loss of skin elasticity to weight, fluctuations or age.The second is excess subcutaneous fat.The third is excess glandular breast tissue.At age 55 you have loss of skin elasticity, and by your picture it looks like a modest amount of subcutaneous fat.It looks like the excision of breast tissue was done fairly aggressively.I’m not sure what you were hoping for but unless the skin component is addressed, there’s always going to be some fullness.To differentiate soft tissues versus skin try taking a look at your chest or have someone take pictures of your chest with one arm at a time up over your head like you’re scratching your upper back.By putting your pectoralis muscle and skin under tension, you can see the remaining to variables.Maybe your candidacy for the procedure was not as great and this may not have been explained to you during the consult.Best,Mats Hagstrom MD

It looks to me like you have a small gland on either side . . . there is a contour pooch on either side seen laterally. Surgery can help the contour, but your areolas may not shrink in diameter as much as you would like . . . the amount of volume removed will be subtle.
