Raised puffy donut skin around incision? (Photo)

hello, i had gynecomastia surgery 6 months ago and while mostly it is healing well - i have one incision next to each nipple where the liposuction drain was - they use prolene sutures that had to come out a few days after surgery and at first the incision was flat and it still is, it’s actually almost white, but the skin around the incision scar is raised in a circle donut fashion? i can’t find anything online about why this may be, do i have any options to treat this raised hard skin?

Hi Graceful461044. Thank you for your question regarding raised puffy donut skin around the incision after gynecomastia surgery.

Thank you for your history and your picture. It appears from your picture that the raised hard skin you are focused on is scar tissue. Scar tissue can be treated with injections of Kenalog, corticosteroids, or a product called 5FU. These products can help to break down scar tissue. You may need serial injections every six weeks.

One of the problems with steroid injections is that they can cause hypopigmentation, meaning this area will get whiter. Another possible downside of the injections of steroids is the atrophy of the tissue underneath the injection site, which could lead to an indentation. Another scar treatment can be microneedling, which can help to break up the scar tissue. Consider these modalities if the scar concerns you.
