Thank you for your question. In the operating room we see vaginas and blood every day. It's nothing to be worried about. The nice thing about surgery is that everyone sees all body parts every day! We all have them! There is nothing sexual about it and nothing to be embarrassed about. It's a normal part of being a woman.

You will be nude during your procedure, though you will be draped appropriately. The underwear would interfere with prepping/cleansing the skin for surgery. It will not be an issue if you are having your period. I encourage you to speak with your surgeon about your concern. It is important that you are comfortable and confident as you prepare for surgery.
