Hi! This is a very common question. The dimpling that you see when you smile is likely due to the muscles of your chin contracting and "dimpling" the attached overlying skin. This is best treated with a neuromodulator (botox, disport, etc) to soften muscle tone in that area. This is a very normal thing to observe and is likely not the result of your previous fat grafting surgery. Morpheus is a great instrument to help with skin tightening in areas with loose skin but won't help the dimpling that is seen as a result of muscle movement!

1. Botox if its muscular- if the pebbling is caused by contraction of the Mentalis muscle, a few units of botox would relax this. Treatment to be repeated every 3-4 months.
2. RF microneedling/ Microneedling if its acne scarring- this can break down scarring that may be tethering down the skin.

I encourage you to visit with your surgeon for an evaluation as you mentioned having fat grafting in the past. If it is determined that the look that concerns you is from muscle activity then Botox provide improvement by softening that effect.
