Is there much difference my breasts could retract whether I leave the capsule or remove the anterior side of it? (Photos)

The technique I recommend is explantation and lift using The Bellesoma Method.  The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery.  Later, fat transfers can be performed if additional volume is desired.  The capsule will be removed if there is indication such as infection or calcification.  Capsule removal does not change skin contracture.     Best Wishes,Gary Horndeski, M.D.

Calcific capsules or capsules that are distorting anatomy should probably be removed during the implant removal procedure.  Leaving a capsule behind is preferred if it is thin and pliable.

as long as you don't have capsular contracture or thick capsules, then you can get only implant removal. In case of thick capsules, implants and capsules should be removed together. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

Thank you for sharing your question and photographs.  There will not be any difference in the degree of tissue retraction based on whether you remove some of the capsule or leave it all in place.  Hope this helps.

You might consider exchanging your implants for something smaller that fits your frame and tissue, and having an internal tightening of your pockets. Remember, you were not happy with your breasts prior to surgery, and may not be happy with the removal and now stretched out tissue and skin. The breasts will still contract some, but not completely, with and without the capsule.

If the capsules are thickened they should be removed along with the implants in order to minimize distortion after surgery. If they are thin then it is your option to have them removed or leave them behind. The breast tissue will still retract whether or not the capsules are present.
