50 yr old desiring smaller, more natural looking breasts without new implants. What happens to the capsule after aspiration - would this increase risk of fluid accumulation and infection? How long can aspirated implants remain before explant surgery? Can mastopexy be performed with explant if 3-6 mos has passed since aspiration giving us better indication of true breast condition?

Answer: Aspirate implants then explant and lift Yes, you can aspirate them in the office and within a day or two you will be able to see your new size. I recommend explant and lift as soon as possible after. There is no advantage in waiting. The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. The capsule is not removed by aspiration and often times not removed during the mastopexy technique.
