Hello, thank you for your question. You are correct in saying that a lift would be necessary to help match both sides. I would discuss the plan over with you plastic surgeon to go over options and placement of scars.

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

You will need a lift since you have some sagging breast tissue, but you could probably do the peri-areolar lift. Measurements are needed to be certain. to make the areola smaller, a scar around the areolae will be made which is the same scar for the lift. If you choose no lift, you will be bigger, but still sagging.

I would recommend a periareolar mastopexy (includes areola reduction) and implants. You should consider moderate profile implants if you are not looking for the size or projection of the full profile implants. Good luck!

You are a very good candidate for an augmentation mastopexy (implants and a lift). I would suggest smaller silicone implants and a periareolar (around the nipple scar) mastopexy on the right, and possibly the same on the left (you might require a lollipop lift, not sure).
