Hello there, greetings from Istanbul, Turkey and thank you for your questions! There seems to be some sagginess on the breasts, but the best assessment would be done on the surgery day in person. For now, the point is to place the desired size prosthesis on the muscle and to look at the final shape, and see how much of a lifting would be ideal in order to provide a lift by making an incision around the periareolar breast. If there is a sagging in the lower part, it can be removed with a small incision.
Also for your case, tear drop / anatomical implants is not recommended. The reason is that the upper part of the breast is empty, this place will remain empty in the tear drop implants and there is a risk of the tear drop implants turning inside the breast after a while. A natural outcome is still achievable with round implants.

Your breasts are low on the chest wall. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers or small implants can be placed totally submuscular.

At your age you might not be crazy about the scars and you have enough breast volume. Might be best to wait until after you have children

Thank you for your question and posting appropriate pictures. Seems you’re a very good candidate for presentation using an internal lift with or without anatomical implants. Ideally and especially if you want your areola size to be smaller you can undergo periareolar lift with a scar just around the areola the same time. Of course it is best to have a virtual consult with 3-D imaging to discuss your goals.

Bases on your photos and your age, I would not recommend any type of breast lift. I think an implant alone can give you a nice look. I would also only recommend a smooth round implant. You will not see much difference with an "anatomical" implant and there are downsides with those. Best to see a board-certified plastic surgeon for an in-person consultation.

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

For you at your age, I do not recommend getting a lift. Implants would look nice, but please do not go too large. I prefer the smooth round implants placed behind the muscle for better upper breast fullness, better coverage, less sagging long term, and fewer long term risks.

Based on your photos and what you have written I think you would best be served with a smooth, round silicone/saline implant. I would not recommend an anatomical implant in your case for fear that it would not give you the fullness/contour in the upper pole of your breasts. I also do not think you need a lift based on your photos.
