I am planning a tummy tuck. I would like to add breast work as well to my surgery. I do not want implants. I am overall happy with my size, however I would a little more perkiness and perhaps more upper fullness and cleavage. I would like to even out my breast size, how does that effects the procedure? Would I Benefit from a lift, and if so, what kind. I understand not all the fat survives in a transfer, but do you feel I would be a good candidate?

Hello and thank you for your question. In an appropriately selected patient, combining breast lift and fat transfer with a tummy can tuck can be an excellent approach. A tummy tuck routinely discards excess skin and fat, the latter of which can instead be harvested for transfer elsewhere - in this instance to your breasts. Nothing is therefore wasted and use of your own fat tissue is optimised to try to achieve the outcomes you desire. To achieve breast volume symmetry, your bigger side could be reduced or more fat could be added to your smaller side. Type of breast lift (for example a circumareolar approach or vertical scar) depends upon a careful assessment of multiple factors including your breast glands, nipple-areolar complex position and soft tissue quality. Make sure you choose an experienced surgeon to assist you in this process which requires careful, good judgement.

Thank you very much for your question, if you can do all those procedures at the same time. the procedure that I recommend in your case:LiposuctionThe surgical procedure of liposculpture,may or may not be associated with technology. Many times the characteristics of these lumps under the skin or retractions are secondary to a conventional fat extraction technique. We handle fat from the use of laser technology, which is light, or vaser, which is internal ultrasound associated with microair (PAL- Power ssisted liposuction) the first two devices allow us to vase or Laser produce lipolysis (is to pass the fat of a solid state to a liquid state so that there are no irregularities under the skin) and the microaire or the PAL allows us to make the extraction of the melted liquid without so much trauma.The problem in your case, is that not only has the sequelae of the lipo, but besides, I think you can have a little diastasis of the abdominal muscles secondary to pregnancy.It is important to be able to evaluate the case a little more to determine if only with a new lipolysis from the combination of technology can be improved. Breast lifts (mastopexy) are a powerful operation to help improve breast shape and overall position (when breasts have deflated and fallen inferiorly). After massive weightloss or multiple (sometimes even 1) pregnancy and breast feeding, the skin has stretched out. Breast lifts help reshape the breast by removing the skin excess and bringing your breast tissue back up on to your chest wall. In some cases, your native amount of breast tissue may not be enough to provide you the volume you're looking for. Options for augmentation with breast lift include implants or fat grafting. Fat grafting is a technique that can be added with breast lifts to give a mild to moderate amount of volume / superior pole fullness to the final result. At best, it provides upto 1 cup size improvement, but should not be considered in cases where patient's are looking for a true augmentation. Fat grafting may have variable take between the breasts and are also associated with different complications compared to implant augmentation. This includes oil cysts, fat necrosis (firmness), and sometimes infection. Proper technique including fat preparation and volume of injection can help minimize these complications. I believe fat transfer with a breast lift is a great option for patients who want both a lift but slightly more fullness to the breast.

It is common to do breast and body work at the same time. I would recommend the lift to get you more symmetric. Fat grafting is commonly done at the same time as a lift and would be a reasonable choice - understanding fat grafting does not give the same results as implants. Best of luck. Hi, thanks for your question. Yes, you can combine a breast lift with a tummy tuck. These procedures are often done at the same time in order to reduce recovery times and avoid having to have surgery multiple times. That said, typically we do a breast augmentation with fat transfer, and yes some of the fat is lost but we compensate for this loss during surgery, so you don't have to be worried about the final result. I think you would be a good candidate for this procedure. That said if you are thinking about a breast lift, you might be thinking about a breast lift with auto augmentation where the patient's own breast tissue is used to improve the shape of the breast. This procedure allows you to achieve more upper pole fullness without adding to the overall volume of the breast. I highly recommend that you book an in person consultation with a board certified plastic surgeon in order to determine which procedure would be best for you and the result that you are looking to achieve.

Ok my thought would be fat graft for volume correction and shaping of the breasts.  Then I actually would recommend InMode scarless breast lift to balance and adjust the lateral position of the nipple/areola areas.  Fat graft done in the right hands with the right instruments is probably 50-60% take rate.  Consider going to a surgeon who is comfortable with fat grafting similar to what is done with Brazilian Buttlifts. I perform the breast lift and TT almost every day and it is appropriate for healthy patients as it saves on cost of anesthesia and involves only one recovery. Performing the breast lift and breast fat transfer would depend upon the type of breast lift performed and the areas into which fat is injected. Staging is probably more reasonable. Fat graft take should always be optimized with the best blood supply available.
