You could do a vertical lift to both reduce your areolar size and lift your nipples. There would be a vertical scar and some component of an inframammary fold scar as well.

At this time, I recommend a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, the implants can be placed higher.

Yes, I agree with your surgeon, Your breast bases doe sit somewhat low and they cannot be raised. If lifting is attempting, your breast will just slowly settle back down. You can relift to tighten the skin and revise the areolae with the vertical lift which will avoid stretching the areolae again.

If you want steep breasts with smaller nipples you may need to consider having a breast lift. I would recommend not touching the implant but just do the lifting with an inverted-T technique.

Hi and welcome to our forum!From your photos, I note relatively large symmetric breasts, associated with large areolae. There is a moderate degree of sag and "lateral boob". The size of the implant plays a role in the shape of the breasts and the areolae as the extra weight and volume of the implant will tend to result in a degree of sag as the skin stretches to accommodate the implant. At 5'2", a 415 cc implant may prove somewhat large for your frame. A breast lift will elevate the nipple areolar complexes back into anatomic position and will conify the shape of the breasts. An areolar reduction can also be performed. It may be wise to consider a smaller implant to prevent recurrence of this phenomenon.Best wishes...
