determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

The problem with the lollipop lift is that it only addresses the loose skin with a vertical incision. To properly address the problems that come with excess breast skin/tissue laxity requires a horizontal incision. This would complete the anchor type pattern breast lift that you truly need.

Unfortunately, you have pseudoptosis. The nipple is above the inframammary fold but you have breast tissue hanging down, laying on the chest wall and excess below. At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

Thank you for your question. Many years ago, I pioneered a technique specifically for patients like you termed "Reductive Augmentation". This involves an internal bra-capsulorrhaphy, mastopexy-lift, and excision of the lower pole breast tissue. You should be able to have a beautiful appearing breast with this technique. You have a perfectly fine result although maybe a bit large in my opinion. I would hold off on any surgery and try to figure out what exactly you’re not happy with .

Please resubmit your photos in the upright position. However, what you have appears to be too heavy for your tissue. I would suggest going with something smaller placed behind the muscle and another lift. Sagging breasts, even if lifted, will not support heavy implants.

Based on your nicely posted photos, I see the two issues you need release of the double bubble skin to muscle junction in the inferior quadrants in an open release and a donut mastopexy possibly? Best of virtual concert with fees to rain from $8000-$15,000..

To Make an accurate assessment regarding the outcome of any plastic surgery procedure we need to see proper before and after pictures.
If you don’t have before, and after pictures, then ask your surgeon to forward the pictures they took.
Patients who’ve had more than one procedure should ideally include before and after pictures for each of their previous operations.
Breast augmentation outcomes are generally on three variables.
The first is patient candidacy. The second is implant selection in regards to size shape and type.
The third is the surgeons ability to put the implant in the correct anatomic location.
It looks like your breast are fairly large to start with and I think Some sagginess would have developed whether you had implants or not.
This is often the case for women who have larger breast.
Some of this is based on genetics.
The surgeons ability to put the implants in the correct and atomic location has a lot to do with the outcome, especially long-term.
If the skin is supporting the gravitational pull and weight of the implant and the skin is more likely to stretch.
If the implant is placed, so it’s supported by the pectoralis muscle then gravitational pull of the implant is less likely to have an impact on the breast skin.
It’s not always easy to second-guess the exact reason for some peoples outcomes.
It would be important to know the placement of the implant and at this point I think you can either have a more aggressive breast lift or accept the outcome the way it is.
