365cc & 345cc then lollipop lift for stretched skin - do I need another breast lift? (Photos)

Question

Back story: I had a breast augmentation when I was 22 years old and my implants never dropped and I had to get them surgically dropped (Sept 2022) These were 365cc and 345cc (two diff sizes). I then had to have a loli pop breast lift (July 2023) after the surgery when they were dropped because i believe it stretched out my skin so bad that I got waterfall deformity. I am sad and feeling defeated. I have not even had kids and my skin is so stretched. What else can I do? Should I give up? Help!

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

The problem with the lollipop lift is that it only addresses the loose skin with a vertical incision. To properly address the problems that come with excess breast skin/tissue laxity requires a horizontal incision. This would complete the anchor type pattern breast lift that you truly need.

Unfortunately, you have pseudoptosis.  The nipple is above the inframammary fold but you have breast tissue hanging down, laying on the chest wall and excess below.  At this time, I recommend explantation and lift using The Bellesoma Method.  The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.  Later, fat transfers or small implants can be placed if additional volume is desired.

Thank you for your question. Many years ago, I pioneered a technique specifically for patients like you termed "Reductive Augmentation". This involves an internal bra-capsulorrhaphy, mastopexy-lift, and excision of the lower pole breast tissue. You should be able to have a beautiful appearing breast with this technique. You have a perfectly  fine result although maybe a bit large in my opinion. I would hold off on any surgery and try to figure out what exactly you’re not happy with .
