Question

Are my implants bottoming out? I got them done August of 2022. At first I was so happy with them. Now they have taken a tube shape. I made the mistake of not wearing a bra for a long time after getting them done (3 months after surgery I went braless except to work). I have gained weight, but I'm not sure why they are looking the way they do. I have lost my confidence.

Dear pwatford,
bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions, which is why I try not to use the inframammary approach to any breast surgery.
Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies.
If you are concerned, please check it with your plastic surgeon for reassurance and to further discuss your options and possibility of revision.

It is difficult to confirm a diagnosis as your photos are not ideal. The photos are suspicious for mild bottoming out. This should be confirmed by a return visit to your surgeon who can discuss treatment options, if any.

Your chest wall is curved. What happens is the implants fall down and out. In your case, a lot of it is falling laterally, your breasts are diverging, your nipple-areola complexes are pointing outward and the cleavage is being lost. To correct this, you will need smaller high profile implants with lateral and inferior plications.

Thank you for your question.Regarding your photos, you have a slight bottom out deformity. It seems that your implants are just a bit inferior to the fold. Smooth implants are more likely to malpositioning after augmentation. Wearing a post-surgical bra day and night at least 2-3 months will help to reduce this situation. this deformity can be corrected by fixing the breast fold.

You do have a double bubble, especially on one side. Not wearing a supporting bra during your healing phase was not wise. Please go back to your surgeon.

To make quality assessment, we need quality information.
To make an assessment regarding the outcome of a plastic surgery procedure we generally need to see a complete set of proper before and after pictures.
If you don’t have before, and after pictures, then ask your provider to forward the pictures they took.
 Your breast have significant divergence and I think this is contributing to your outcome.
It’s really hard to make a good assessment based on the information provided.
I suggest you follow up with your plastic surgeon or schedule if you in person consultations with other plastic surgeons in your community.
