I hadn't implants done 18 years ago. I was AA before the implants (only wanted B-C but ended up DD), and I have breast-fed one child. I've got plenty of fat on my thighs and belly. Do you think I would be a good candidate for a fat transfer at the same time to maintain some of the size? I'm worried about feeling bad about myself with significantly smaller breast (and 40lbs more weight on my frame) and would hope to get to a B or so.

The removal of breast implants is a complex and specialized procedure that requires a high degree of expertise especially if it's done with mammary reconstruction, which in my opinion gives the best aesthetic result and is the right approach. In this case the incision would be periareolar and inverted J-shaped to avoid making a T. A permanent alternative is to augment the volume of your breast with your own fat. However, it is prudent to wait approximately 3 months after the explant. Take into account that the best course of action is to consult one or several board certified plastic surgeon for more information and options tailored specifically to your needs. Best wishes! Answer: Explant with Bellesoma Method The technique I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.
